Use this screen to tell us || how you would like to receive benefits: by direct deposit into || your bank account or on a reloadable, prepaid debit card. ||

If you choose direct deposit, your bank || must have a physical office in Ohio. This is required by state || law. If your bank does not have a physical office in Ohio, any || benefits you're eligible for will be paid by debit card. ||

To choose direct deposit: Select || the button for "Direct Deposit." Next, enter your routing || number. This can be found in the bottom left of the checks || in your checkbook. Then enter your account || number. This can be found in the bottom middle of the checks || in your checkbook. If you don't have paper checks or can't || find your routing or bank account numbers, contact your bank. || Next, select your account type from the "Account Type" || drop-down box. Then click "Next" to continue. ||

To choose debit card: Select || the button for "Debit Card." Skip the || remaining fields and click "Next" to continue.
