In the first question under this section, choose "Yes" or "No" to indicate whether you have an actual date to return to work. If "Yes", then enter the date and the last day you worked using two digits for the month and day and four digits for the year. If you expect to be recalled by any former employer within the next 12 weeks, select "Yes". Otherwise, select "No".

Choose "Yes" or "No" if you are attending school, college or vocational training. If you answer "Yes", complete the follow-up questions. In the next question, indicate whether you were in school during the period which will be considered in your unemployment claim.

If you are a member of a skilled trade union, select your union name from the first combo box. Then, answer the rest of the union-related questions.
